Bid Form s cope ID #01 -001 December 3, 2010 <br />' Cabarrus County Board of Elections <br />Concord, North Carolina <br />Final Cleaning <br />t The undersigned Bidder, S R ,( �-_ `- s k t -e C LO I (,I 3-9- f' --p _ having <br />carefully Investigated the existing conditions at the project site, and having thoroughly familiarized himself <br />with the Documents contained In the construction drawings & project manual, December 24, 2010 as <br />' prepared by Yates- Chreitzberg- Hughes Architects, 145 Union Street, South, Concord, NC and its <br />consultants, the CM Black Bid Package, dated December 3, 2010 hereby proposes to provide all <br />necessary labor, equipment, materials, services, etc., to complete f=inal Cleaning (Supplemental <br />General Conditions) as defined within the Summary of the Work and all other parts of the Contract <br />' Documents. Project is located on 369 Church Street North in Concord, NC on property owned by <br />Cabarrus County. All work shall be accomplished in accordance with the aforementioned Contract <br />Documents with a Lump Sum Price as follows: <br />' BASE BID: (including Taxesdk4k _).3gn4 0- 6J e W ToP 1'r fIc pollars 6 9 S 51 _� <br />' <br />BASE BID BREAKDOWN -- (Price Included in Base Bid) <br />Preliminary Cleaning Bid 0 C� <br />' <br />Final Cleaning Bid $ �(� . q3 <br />Touch -up Cleaning Bid $ 0 . <br />' <br />The undersigned further agrees that this proposal shall be valid for a period of Sixty (60) days from the <br />date of receipt for the bids and that if this proposal is accepted by the Owner and Construction Manager <br />within this period, the Bidder will execute the contract form and provide surety bonds (if applicable) as <br />' <br />described In the Contract Documents. <br />For contracts over $50,000, the undersigned further agrees that in the case of failure on his part to <br />execute the said contract and the bond within ten (10) consecutive calendar days after written notice <br />being given of the award of contract, the certified check, cash or bid bond accompanying this bid shall be <br />paid Into the funds of the Owner's account set aside for the Project, as liquidated damages for such <br />failure; otherwise the certified check, cash or bid bond accompanying this Proposal shall be returned to <br />the undersigned. <br />It Is expressly understood that the Construction Manager is expected to act as project expediter and <br />coordinate work of all other Subcontractors. <br />' We estimate North arollna Sales Tax included within our above designated Base Bid and /or Alternates <br />to be$ <br />Accompanying this proposal Is a bid security (5% of Contract Sum) in the form of. _ <br />' (for contracts over $50,000.)
