1 <br />r <br />7 <br />i <br />Bid Form Sco peID #09 -001 January 24, 2011 <br />uaoarrus Uounty Board of Elections <br />Concord, North Carolina <br />Metal Stud Framing / Drywall <br />The undersigned Bidder (Company Name) <br />having carefully investigated the existing conditions at the project site, and having thoroughly familiarized <br />himself with the Documents contained in the construction drawings & project manual, dated December 24, <br />2010, as prepared by Yates- Chreltzberg- Hughes Architects, 145 Union Street, South, Concord, NC and Its <br />consultants, the CM Black Bid Package, dated December 3, 2010 hereby proposes to provide all necessary <br />labor, equipment, materials, services, etc., to complete the Metal Stud Framing, Drywall and Corner Guards <br />(Supplemental General Conditions and Divisions #06400, # 07200, #07840, #09280 and #10260) as <br />defined within the Summary of the Work and all other parts of the Contract Documents. Project is located at <br />369 Church Street North In Concord, NC on property owned by Cabarrus County. All work shall be <br />accomplished In accordance with the aforementioned Contract Documents with a Lump Sum Price as follows: <br />BASE BID: (Including Taxes) __ <br />Dollars (S t �bQ <br />BASE BiD BREAKDOWN (Price Included In Base Bid) <br />Gypsum Drywall & Metal Stud (09260) (06400) <br />Bid $ __ t d . 74:3 <br />Firestoppfng (07840) <br />Bid $ 3 <br />Insulation (07200) <br />Bid $ 4 <br />Corner Guards & Frame Protectors (10260) <br />Q <br />Bid $ 1j <br />The undersigned further agrees that this proposal shall be valid fora period of Sixty (60) days from the date of <br />receipt for the bids and that if this proposal Is accepted by the Owner within this period, the Bidder will execute <br />the contract form and provide surety bonds (if applicable) as described in the Contract Documents. <br />For contracts over $60,000, the undersigned further agrees that in the case of failure on his part to execute the <br />said contract and the bond within ten (10) consecutive calendar days after written notice being given of the <br />award of contract, the certified check, cash or bid bond accompanying this bid shall be paid Into the funds of <br />the Owner's account set aside for the Project, as liquidated damages for such failure; otherwise the certified <br />check, cash or bid bond accompanying this Proposal shall be returned to the undersigned. <br />It is expressly understood that the Construction Manager is expected to act as project expediter and coordinate <br />work of all other Subcontractors. <br />We estimate North Carolina Sales Tax included within our above designated Base Bid and /or Alternates to be <br />Accompanying this proposal Is a bid security (5% of Contract Sum) in the form <br />(for contracts over $50,000.) <br />Alternates: <br />Should any alternate be accepted, the amount written below shall be the amount to °add to" or 'deduct from' <br />the appropriate Base Bid. Refer to Section 01030 — Alternates, the Drawings and the CM Black Bid Package <br />for complete description and delineation of alternate requirements as the descriptions below are not Intended to <br />fully convey the scopes of work.
