Bid Form Scope ID #10 -004 December 43, 2010 <br />L;aaarrus younty Board of Elections <br />Concord, North Carolina <br />Fire Extinguisher Cabinets <br />I <br />The undersigned Bidder (Company Name) _ C ATi AM UO � 1+4V A?-G I ( <br />having carefully Investigated the existing conditions at the project site, and having thoroughly familiarized <br />himself with the Documents contained in the construction drawings & pro)ect manual, dated December 24,2010 <br />as prepared by Yates- Chreitzberg- Hughes Architects, 145 Union Street, South, Concord, NO and Its <br />consultants, the CM Black Bid Package, dated December 3, 2010 hereby proposes to provide all necessary <br />labor, equipment, materials, services, etc., to complete the Fire Extinguisher Cabinets (Supplemental <br />General Conditions and Divisions #101321) as defined within the Summary of the Work and all other parts of <br />the Contract Documents. Project Is located at 3139 Church Street North in Concord, NO on property owned by <br />Cabarrus County. All work shall be accomplished In accordance with the aforementioned Contract Documents <br />with a Lump Sum Price as follows: <br />1 <br />H <br />BASE BID (Induding Taxes) Dollars ($ I ��• a`1 i <br />The undersigned further agrees that this proposal shall be valid for a period of Sixty (50) days from the date of <br />receipt for the bids and that if this proposal is accepted by the Owner within this period, the Bidder will execute <br />the contract form and provide surety bonds (if applicable) as described in the Contract Documents. <br />For contracts over $50,000, the undersigned further agrees that In the case of failure on his part to execute the <br />said contract and the bond within ten (10) consecutive calendar days after written notice being given of the <br />award of contract, the certiffed check, cash or bid bond accompanying this bid shall be paid Into the funds of <br />the Owner's account set aside for the Project, as liquidated damages for such failure; otherwise the certified <br />check, cash or bid bond accompanying this Proposal shalt be returned to the undersigned. <br />It Is expressly understood that the Construction Manager is expected to act as project expediter and coordinate <br />work of all other Subcontractors. <br />We estimate North Carolina Sales Tax included within our above designated Base Bid and /or Alternates to be <br />$ i2-31 <br />7 <br />Accompanying this proposal is a bid security (6% of Contract Sum) In the form of: <br />(for contracts over $50,000.) <br />Alternates: <br />Should any alternate be accepted, the amount written below shall be the amount to "add to" or "deduct from' <br />the appropriate Base Bid. Refer to Section 01030 — Alternates, the Drawings and the CM Black Bid Package <br />for complete description and delineation of alternate requirements as the descriptions below are not intended to <br />fully convey the scopes of work.
