SIX MONTHS MEASURABLE OBJECTIVES STATUS <br /> <br />(Continuation Programs Only) <br /> <br />In the space provided, please list each measurable objective in your em-rent program agreement and indicate the <br />degree to which your program has been successful in achieving them. (Use data from the first six months of the <br />fiscal year.) <br /> <br />Measurable objectives met July 1, 1995 through December 31, 1995. <br /> <br />· 85% referred children served <br /> 84% - 16 of 19 children referred were served. Two continued in their present placements and need was <br /> alleviated. One was referred but was on mn and other plan~ were made. Three were already in placement <br /> on 7/1/95 giving a total of 15 served during first six months. <br /> <br />60% prepared for long term placement <br />64% - 9 of 14 returned home, placed in foster home or group home or placed with relatives as determined <br />to be the best long term placement. <br />1 placed in group care, kicked out <br />2 were placed in foster homes and ran away. <br />1 ran and was not located for 30 days, went to sister's. <br /> <br />50% achieved successful adjustment for six months <br />50% - 7 of 14 children placed between September 1, 1994 and March 31, 1995 have had no new court <br />referrals and have made successful long term care placements. Four of the 14 children terminated had <br />additional court charges. Five had multiple placements. Four were out of school on suspension. Of <br />the 7 successfully adjusting to their placements, 5 are at home. <br /> <br /> Bctw..cen 7/1/94 and 6/30/95, we had 20 children placed in the group home; 2 were for less than 30 days, <br />2 for 30 to 60 days, 7 for 60 to 90 days, and 5 for the maximum 90 days. Four were for extensions of maximum <br />of 120 days. If'we have 5 children stay for 90 days our maximum is 20 children served. <br /> <br /> From 7/1/95 through 3/5/96, we had had 20 children placed. Currently there are 3 children placed in the <br />group home as of 3/5/96. <br /> <br /> In the CBA 4th quarter report of FY-95, our statistics for the past 12 months showed 88% referred by <br />court, 88% referred by court, 88% referred as delinquent or status offenders. Our performance indicators <br />showed 81% decrease in additional court referrals, 60% decrease in runaways, and 28% decrease in school <br />suspension/expulsion. 77% were between the ages of 14 and 17; 66% were white, 33% black; 55% female, 44% <br />male· 88% were active with Juvenile Court, 82% were on probation or court supervision, 5% had a petition filed <br />and 11% were at risk of Juvenile Court involvement. 68% had successful completion of the program. 6% were <br />removed by court, 12% because runaway and 12% because the did not adjust. At time of discharge 87% were <br />enrolled in school, 31% had had court referrals while placed at the group home. 37% had had at least one <br />runaway, 56% had school suspension or expulsions and 18% had been in secure custody. At discharge 62% <br />went to relatives or hack home, 24% went to group home or foster home, 6% went to other living arrangements <br />including hospitalization and 6% went to training school. <br /> <br /> <br />
