Statement of the Problem [continued]: <br /> <br />difficulties by child, lack of community resources, rapid population growth, number <br />of juvenile petitions and training school commitments create a need for an out-of- <br />home placement for children in this county. <br /> <br />Program Goal [continued]: <br /> <br />in the delinquency/undisciplined juvenile rate being decreased. <br /> <br />Measurable Objectives [comtinued]: <br /> <br /> 6. Ail youths experiencing substance abuse will be referred to substance abuse <br /> screening and follow-up counseling when indicated. <br /> 7. Ail y~uths will be enrolled in a school system and educational assessmerzts <br /> will be done with the school to determine the most appropriate educational <br /> setting for the child. <br /> 8. Sixty percent (60%) of all youths served by the program will be prepared for <br />-- long-term placement within 90 days of the entry date (preparation is defined <br /> as stabilization of the child's behavior to a point where he is ready for <br /> placement and development of a long-term care plan). <br />_ 9. Of the children completing the 90 day program. 50% will achieve successful adjustment <br /> for six months following long-term placement. (Success shall be defined as: <br /> [1] The child does not come to the attention of the Juvenile Court; [2] The <br /> child, the social worker, add his legal guardian agree that placement was success- <br />- ful six months after it was made.) <br /> <br />Information Maintained for Effectiveness Measurement [continued]: <br /> <br />2o <br /> <br />Department of Social Services will keep a folder on each child containing <br />a court order showing custody with Department of Social Services, termination <br />summary on the child, case dictation involving family counseling and coordiantion <br />of services to the child and family, monthly progress reports submitted by the <br />home to DSS, client tracking forms submitted to CBA office. <br />Budget information documenting in-kind and cash flow kept by both the home <br />and DSS. <br />Staff work schedule kept by the home. <br /> <br /> <br />
