<br />Consumer-Directed Services <br />Fiscal Intermediary Agreement <br />Page 2 of 2 <br /> <br />· Provide payroll services on behalf of the Client for hislher personal attendant(s) and make <br />payment to said personal attendant( s) through direct deposit into the bank account provide. <br />· Report and pay employment related taxes on behalf of the Client, as well as the Client's <br />personal attendant(s). <br />· Provide the Client with the appropriate documentation to accurately process and file annual <br />income taxes. <br />· Provide each personal attendant an IRS W-2 form to properly report hislher income from the <br />Client. <br /> <br />General Provisions: <br /> <br />· This agreement is effective beginning on the last date specified in the signature section and <br />will remain in effect as long as the Client remains active in the Cabarrus County Consumer- <br />Directed Services program. <br />· Cabarrus County, as represented by Aging and/or Finance is not the employer of the Client, <br />or any personal attendant that is hired by the Client to serve as a household employee. <br />· Cabarrus County, as represented by Aging and/or Finance does not in any way control the <br />employment environment and/or situation associated with the services provided to the Client <br />by the personal attendant(s). <br />· Cabarrus County, as represented by Aging and/or Finance assumes no responsibility and/or <br />authority regarding the employer/employee relationship between the Client and the personal <br />attendant(s). <br /> <br />SignaturelTitle of County Representative <br /> <br />Signature of Consumer-Directed Client <br /> <br />Printed Name <br /> <br />Printed Name <br /> <br />Date <br /> <br />Date <br /> <br />G~~ <br />
