"Approval for Temporary Impoundment" was issued on March <br />3, 1994; "Approval for Temporary Impoundment - Addendum No. 1" <br />was issued on June 29, 1994; and "Approval for Temporary <br />Impoundment - Addendum No. 2" was issued on October 21, 1996. <br /> <br /> "As-built" drawings were received on December 20, 1993. <br />Engineering certification of the dam was received on December 28, <br />1993. The dam was certified by Mr. L. E. Baker, P. E. Repairs <br />to the spillway were certified by Mr. Jack R. Miller, P. E. in a <br />letter dated September 12, 1996. <br /> <br /> Inspections of this dam were made by Land Quality Section <br />staff of the Mooresville Regional Office on January 11, 1994; May <br />11, 1994; February 27, 1996; September 19, 1996; and May 15, <br />1997.
