Most importantly, it <br /> would mean a social worker or case manager checking with the client on a <br /> regular basis, as well as checking with the employer to detemdne how the <br /> new job situation is progressing, and in the case of the employer, offering <br /> the support of the agency. Finally, making the client understand that Work <br /> First staff is there to support them in this new endeavor, and encourage <br /> them to call when they encounter a crisis situation. <br />b. Activities and Support Services to be Provided: The support of the <br /> Work First program is the primary support that is needed in this situation. <br /> Once the client has gone to work, it is our job to ensure that we provide <br /> whatever it takes to keep them on the job. The lack of reliable <br /> transportation for an extended period of time is a common reason for <br /> clients losing jobs. The additional funding in the amount of $14,000.00 <br /> through the NC Department of Transportation can assist in this. <br />c. Who Will Manage the Support and Activities: The social workers, <br /> case managers, or other designated staff will be responsible for providing <br /> the support to the newly employed chents. <br />d. Where Will This Take Place: It will take place wherever it is needed. It <br /> can range anywhere from phone calls, to home visits, to employer visits. <br /> Also, it many be necessary to assist the client in accessing other support <br /> from other community agencies. <br />e. How We Will Measure Outcomes for This Goal: Presently there is no <br /> tracking tool for this goal. We plan to use the JTPA method of tracking, <br /> which involves calling the client at intervals of 30 days, 6 months, and 1 <br /> year, to determine if they are still employed. <br /> <br />Meeting the "All Parent" Participation Rate: <br />How Will This Goal Be Met: Presently the participation rate <br />requirement is 30%. Effective October, 1998, it increases to 35%. In <br />order to meet this goal, intensive efforts must be made to assure that as <br />many clients as possible are participating at a minimum of 35 hours per <br />week. Work sites for Work Experience clients will need to be developed, <br />to supplement the working hours, when a client cannot find full-time paid <br />employment. Staffwill need to monitor job search to ensure that this <br />activity is intensive enough to bring hours up significantly. It will require <br />good case management, and in many cases, it will require a combination <br />of hours fi'om full and part-time employment, work experience, and job <br />search to meet the minimum participation hours. A pay for perfomiance <br />system will be necessary to meet this goal. <br /> <br />9 <br /> <br /> <br />
